Essential Elements of Transition Planning

Transition Planning has several essential components that must work together in order to achieve the optimum post school outcomes. These elements include; transition assessment, transition planning & IEP, family involvement, student involvement, curriculum & instruction, inclusion and interagency & community services. Special Education services for students with disabilities is devoted to facilitating coordination of these components for a seamless transition to the adult community.
